Anthem - Specialty Pharmacy and Copay Cards
Hi,
I'm trying to reduce the chances that the insurance company not counting the copay card payments toward my deductible or out-of-pocket maximum.
Does using their specialty pharmacy make that more likely? In other words, are they more likely to side with the insurance company?
I'm with Anthem through the exchange, and their specialty pharmacy is Carelon.
I read that accumulator programs have been banned in Nevada starting in 2025, but even the representative I spoke with at Carelon said that's not the case.

Cigna - Hospital bill should cover out of pocket max but two months later claim isn’t processed
My wife recently had sinus surgery, and we paid a $3,000+ hospital bill before the surgery as they said she couldn’t have the procedure without paying it. That covered the rest of her OOP max. Cigna still hasn’t processed the claim two months later, and meanwhile other bills and late fees are stacking up because her OOP max isn’t shown as being met.
We contacted Cigna and they said it finally processed last week (not reflected on their website) and that we need to allow 21 days for it to go through. How do we handle this with other providers calling and threatening additional late fees? My wife wants to pay but I think we should wait for the claim to go through.

Choice Home Warranty - Hope warranty company refusing to provide proof
Location: Virginia
I use Choice Home Warranty (I didn’t realize how scammy home warranty companies were until recently, so please don’t comment about that). They are requiring I pay a portion of a repair because it’s considered a “modification.” I pointed out that my particular policy covers up to $250 of modifications, and they are saying that that $250 was applied to a previous repair and no longer available. However they will not provide me any written documentation or the recording of the phone call where they stated this. Do I essentially just have to trust them, or are they required to provide proof?
All State - Help me get my basement flood covered?
My basement flooded this week with 5 inches of water. We have had a lot of rain. The sump pump appears to have given out and the backup sump pump didn't kick on. This has not fully been confirmed as the reason for the water because when the plumbers arrived the pit was only 2/3 full and water wasn't flowing out. BUT we have not been able to identify another cause or factor for the flood. The plumber mentioned that it could be due to malfunctioning drain tiles. We had an inspection and it was confirmed our house does not have drain tiles.
We have insurance through All State and our policy states the following is **not included.**

I don't think there is much room for negotiating coverage with All State, but posting here because I am **desperate**. Does anyone have advice on what I can look into or how to try get this covered? The assessor is scheduled to come in 2 days and trying to get a case out together. TY!
Blue Cross Blue Shield of Illinois - Why am I paying so much?
My husband and I signed up for BCBS of Illinois PPO+ plan through his work this year. I started seeing a physiatrist who was in network. When my claim was submitted, they only approved a discount from $360 to $219 leaving me having to pay $219 out of pocket. I previously had United Healthcare from my last company and with that insurance my physiatry appointments were only $30. I have read through our policy agreement but have to admit, I have no idea what I am reading. Can someone help explain what is different between my currently BCBS plan that only approves a discount vs other plans who only make you pay the co-pay? Thank you!
State Farm - Lapse in payment
When I turned 18 I too out a universal life insurance policy with State Farm. I took a “loan” out on my policy to pay for school loans when I was about 24. Since then I fell behind in paying back the money and the policy itself.
I was unaware and uneducated on the whole process when I borrowed the money.
Does anyone know if the policy is canceled, do I still owe what I borrowed? Or not considering it was money I had already paid towards my own policy.
It’s crazy that we have to pay back what in reality is out money.
United Healthcare - Sent a bill 13 months later
On March 11th, 2024, I had an outpatient surgery procedure done.
Flash forward to today, April 8th, 2025 and I just received a bill for over $3000 for this surgery. The bill states that the surgery cost overall was $20,000 and my insurance at the time paid for ~$16,000. I was covered under United healthcare and this coverage ended about 5 months ago.
Here are my questions:
1. Why am I just getting this bill now? Is this even legal? (I live in WI)
2. What would be the first step to getting this figured out?
Geico - Insurance wants to total car and offer pennies. Can’t afford to get another vehicle
Title pretty self-explanatory. A lady hit my car in the mall parking lot while I was inside shopping. Car is a 2005 Chevy Malibu with 306K miles on it and a few minor dents and scratches. The damage caused by the lady who hit my car is significantly worse than any previous damage incurred and exceeds the value of the vehicle, so her insurance company wants to deem it a total loss and offer about $2K. I took the vehicle to a body shop to get my own estimate, which came out to be $3.7K. Insurance company doesn’t want to budge on previous estimate.
I get that the car is old and not worth anything, but I’ve learned how to keep it running and can afford its maintenance. I can’t currently afford to get a new or spiffy used car, but I’m also not sure $3K is enough to get a solid cash car that won’t break down on me. I also live in Georgia, where having a salvage title isnt really an option.
What would you do in the predicament? Any advice is appreciated.
